Data Reporting Forms for Modules

There are two data reporting forms for any module that is assigned a report

• Session Lead Form: Filled by the person leading the session. See a Sample for Module 2 here. The exact questions need to be validated by the partner.
• Session Support Form: Filled by the person supporting the session. See a Sample for Module 2 here. The exact questions need to be validated by the partner.

Facilitator can view and edit each parent's attendance and family case profile in the Parent Profile under "Parent Progress"

I have added reporting forms corresponding to the linked forms.
It is set up so the user specify there role initially then the questions change accordingly. When we talked last we said that it can change who leads the session so I thought a simple question at the beginning of the report would work the best.

I will leave the issue open till all reports are confirmed
