Expand the Parent Profile
Parent Profile to include this information.

Users cannot save the Parent Profile without answering the questions marked as required.
If a Parent Profile is incomplete, an incomplete icon asset/signifier should be displayed on the Parent Group homepage.

In the Parent Profile, users can also view the parent's attendance and parent's family case profile (see google form in #15)

Access Permissions:

Editing & Adding: Only Facilitators can add or edit the Parent Profile.
Viewing: Both Facilitators & Co-facilitators can view the Parent Profile.
