2018 version, now with segwit!

This upgrades bitcoind (0.16.0) and Ubuntu (18.04), and switches to use modern P2SH-segwit addresses. The most important change is that you can't make a mistake with the amounts you enter for spending inputs: the signature won't work if you get it wrong.
