Steps to reproduce:

1. Go to encounter page and click any patient
   2 Next click on patient details page
   3 Clock on request tab and create a request

@Rishith25 You are welcoming to address the issues with the form, but let's leave the resource page alone for now - it will be redesigned soon anyways.

@Jacobjeevan due to the merging of PR #9935 , the issue related to creation of resource now solved.

For further enhancement i want to do the following changes.

• Replace deprecated form components with shadcn ui components
• Instead of facility ID , facilityname should be visible in breadcrumps

• Instead of patient id , we can show patient name and add a link
