feat: Improve Discord notifications #3269

Always use next branch as destination branch for PRs, not main

This PR modifies the Discord notification messages to be slightly more detailed (from just the app name, to include environment and project names) as well as "prettify" the message sent.

TODO:

  • Ensure correct variables
  • Apply 'theme' to all the available notifications
    • Successful deployment
    • Failed deployment
    • Status change

Note to maintainers: Images with before/after will be supplied once RTR :)
