This is the manuscript for my doctoral dissertation.

I will update the chapters, front- and backmatter when sections or chapters are near completion, i.e. not continuously while writing. My intention is however to keep updating the ToDo-list with new and checked items (if it proves useful with a ToDo-list and doesn't take too much time to maintain).
