Address problems found by checkers

* Clean up PHPDoc so that static analysis tools can understand them.
* Rewrite how the tag interior is parsed.
* Remove anti-pattern of disabling cache.
* Use Title class to parse tag file names instead of writing our own parser.
* Move all user checking to its own code.
