Use moving window for memory check

The memory check now uses a moving window approach - adding options for window and threshold along with interval.

Don't fail without heroku auth token

Fixes an issue where the HEROKU_AUTH_TOKEN is always expected, whether it's needed or not

Memory Check

Adds a new check, which calls the heroku api check for R14 (out of memory) errors.

Graphite working check now, err, working

Fixes an issue where the API key wasn't being used to contact graphite.

New check for Graphite

This release adds a new check, graphiteWorking, useful for checking the graphite stats are being collected.

Also a refactor of the tests and swapping out isomorphic-fetch for node-fetch instead
