Add extra row of slots and configure default slots

IMPORTANT POSSIBLE BREAKING CHANGE
NOTE: if you do not configure the slots yourself it will use the defaults set in the card. These defaults have been changed from the original defaults so they match the graphic in the readme in this release. This means that you may need to check/edit the lovelace configuration for the slots you are using.
