When a user fills out a survey, the last step is to choose how to sign. There are three possible options (sign as your self when logged in, sign anonymously or sign with name and e-mail). Sometimes only one option is available. In those cases, the user is still prompted to "Choose how to sign", although there is no real choice.

The label (and radio button) could be replaced with a single "Sign" label in these cases.
