Speaking somewhat cryptically to avoid spoilers.

The Lexicon entry "Contract Breaker" does not appear until after completion of the ritual it hints at.
My book gained "Forbidden Knowledge" as soon as I made the final awakened items, but no new entries appeared.

Upon completion of the ritual, the entry appeared alongside the new faith entry.
