You can ask user to select option 1,2,3,4

the time slot cannot be added it will slow down the booking process. plus booking process is automated so selecting time slot idea is not good i think

You can visit center anytime, regardless of time slot you choose during booking. Center is not asking time slot, it will only ask Name and last 4 digit of refence number
