Multiple issues identified in APIM 3.2.1 distribution #2991

Description

We have identified below mentioned issues recently in APIM 3.2.1 distribution.

Ack/Nack issue [1].
ConcurrentModificationException [2].
NullPointerException happening from isAPIThrottled method [3].
MB_DB growing issue due to FOREIGN constraint missing in MB_CONTENT table.

Hence, going to address those via this public GIT issue.
