This will take a user to a form on our ServiceNow environment that is pre-populated with the correct area values to open a ticket in the queue we have set up for Jupyter support. Login with UTORid is required.

We should feature this in two places:

In the hub homepage
In the header of classic Jupyter Notebook

(1) is higher priority, (2) can happen slower.
