Create MILCON ERCIP Capital Cost Page

We need to create a capital cost page specifically for the MILCON ERCIP project analysis type. It will be a unique page because it will include a line item breakdown of the investment costs, each of which must be reported as its own cost object.
Here are the headings. Each has an input (in $) with 2 decimals:

Construction Cost
SIOH
Design Cost
Total Cost (not editable; its the sum of construction cost, SIOH, and Design Cost)

Salvage Value of Existing Equipment
Public Utility Company Rebate
Total Investment (not editable; its the total cost minus salvage value and rebate)

Instead of generating a single E3 object for this cost page, we need to create 5 cost objects, one for each (non-sum) line item. Those need to be provided in the results. See issue related to displaying ERCIP results.
