If this feature request related to a problem? Please describe.

The pacs table needs new fields added (IP, port, AET, Return AET).
IP: for what server to send to
Port: for what server to send to
AET: unique identifier that identifies the pacs, must be unique within our system
Return AET: The AET of the PACS that we would like to return the info to
