Coverage for New Issue Types

As of now, there are certain errors we don't have a UDOIT form to cover. With the implementation of Equal Access as the new default scanner, we see a lot more errors to build forms for, as well as repurpose old forms if necessary.

To tackle the first point, we need to build new forms to tackle these error types.
