Ce rapport utilise des données génomiques partagées publiquement pour suivre la propagation du SARS-Cov-2. Nos rapports sont mis à jour chaque semaine.
# Résumé exécutif

Ici, nous avons analysé 723 génomes du SARS-Cov-2 partagés publiquement. En comparant ces génomes viraux entre eux, nous pouvons caractériser comment SARS-CoV-2 évolue et se déplace dans le monde.

Dans ce rapport, nous soulignons que le virus a été introduit et circule dans de nombreuses régions du globe.
Pour savoir si et dans quelle mesure SARS-Cov-2 circule localement et comment les mesures d’atténuation peuvent ralentir cette propagation, il faut une bonne connaissance de la dynamique locale des épidémies. Cela nécessite à son tour des tests à grande échelle.
Cela souligne l’importance d’un dépistage rapide en mettant l’accent sur le ralentissement des transmissions locales.

Dans les mises à jour de cette semaine, nous rapportons :

* Des preuves d’introductions liées aux voyages dans de nombreuses régions du monde.  
* De nombreuses régions avec des introductions récentes peuvent s’attendre à une augmentation du nombre de cas dans une fenêtre de 4 à 8 semaines. Il faut se préparer maintenant.   
* De nombreux endroits qui ont récemment mis en œuvre des mesures de distanciation sociale peuvent s’attendre à voir des cas continuer à s’accumuler à court terme, mais les nouveaux cas vont probablement se stabiliser et diminuer à moyen et long terme.
* Le dépistage des infections actives et des individus guéris sera vital pour faire face à cette épidémie.

Nous avons préparé quelques ressources qui valent la peine d’être lues pour vous familiariser avec COVID-19 et le virus qui le provoque, SARS-CoV-2. Ces informations faciliteront l’interprétation des données que nous présentons dans ce document. Nous avons actuellement des séquences d'échantillons prélevés dans 36 pays sur 6 continents. C'est un exploit incroyable: séquencer un virus à ARN inconnu de grande taille au milieu d'une pandémie est difficile et n'est possible que grâce au travail extraordinaire et au partage en temps opportun de données par des scientifiques et des médecins du monde entier.

Bien que ces données nous permettent d'inférer de nombreuses caractéristiques utiles de l'épidémie et de suivre sa propagation en temps réel, il est important de souligner que nos conclusions sont limitées par les données disponibles. Sur le nombre total de cas, seul un sous-ensemble de patients est testé. Parmi ceux qui sont testés, seul un sous-ensemble d'échantillons est également séquencé. Le sous-ensemble de cas qui est testé et séquencé peut varier considérablement selon les régions géographiques et le temps. En outre, les reconstitutions phylogénétiques des épidémies sont toujours associées à un certain degré d'incertitude.

Examinons quelques exemples.

La carte ne montre que très peu de séquences provenant du sud de la planète. Par exemple, nous n'avons que des séquences en provenance de seulement 4 des 25 pays d'Amérique latine où l'OMS rapporte les cas de SARS-CoV-2. Ce n'est pas parce que SARS-CoV-2 ne circule pas dans ces régions, ou que ces cas ne sont pas aussi importants à comprendre, mais plutôt parce que nous n'avons pas beaucoup de données disponibles dans ces régions.

Dans d'autres régions, telles que les Pays-Bas, il existe une grande quantité de données de séquence disponibles, par rapport au nombre total de cas.

Ainsi, en lisant ce récit, gardez à l'esprit que la taille de chaque cercle sur la carte indique la quantité de données actuellement disponibles pour cette zone, plutôt que la taille réelle de l'épidémie. Vous pouvez lire ici une explication sur la façon dont cela peut affecter nos déductions sur la transmission.

Identifier où un cas donné a été infecté est important pour comprendre quelles régions ont une transmission locale par rapport aux cas principalement associés aux voyages. Les données de séquence peuvent nous aider à différencier ces scénarios.

Ici, l'arbre est coloré par l'historique de voyage d'un cas (lorsqu'il est connu). En regardant la séquence du Canada (Canada/BC_78548/2020) au milieu, nous observons qu'il y a un historique de voyage en Europe. Cette infection se regroupe avec d'autres séquences européennes, montrant que ce cas est presque certainement une infection liée aux voyages.

En effectuant un zoom arrière, nous pouvons voir que ce n'est pas toujours aussi cohérent.

Ici, nous pouvons observer que presque tous les cas de voyages récents en Iran se regroupent au milieu de l'arbre.

Vers le haut de l'arbre, nous observons également un cas canadien avec un historique de voyage en Iran; cependant, cette séquence ne groupe pas près du reste des cas avec historique de voyage en Iran.

Il est certainement possible que cette personne ait été infectée par une deuxième chaîne de transmission en cours en Iran, mais tant que nous n'aurons pas plus de données, nous ne pouvons pas confirmer que cette infection a été contractée lors du voyage.

Sur l'arbre, nous observons beaucoup de ce genre de mélange d'échantillons de différents pays. Cela indique que les inévitables mouvements humains ont déjà introduit le virus dans de nombreuses régions du globe.

En effet, les rapports de l'OMS ont confirmé des cas dans 159 pays sur 195 dans le monde.

Cependant, toutes les introductions ne donnent pas lieu à des épidémies.

Il peut sembler que SARS-CoV-2 a soudainement explosé. Pour de nombreuses personnes, cette épidémie est peut-être passée d'une préoccupation abstraite dans d'autres parties du monde à quelque chose qui affecte gravement leur vie quotidienne, du jour au lendemain.

Mais gardez à l'esprit que cela se prépare depuis un certain temps. Ce n'est pas nécessairement que le virus est introduit plus souvent dans de nouveaux endroits. Très probablement, de nombreux endroits commencent à prendre conscience des épidémies localisées qui ont commencé par des introductions qui ont eu lieu il y a plusieurs semaines.

Les régions qui n'ont pas encore connaissance de foyers locaux doivent se préparer dès maintenant et commencer les tests de surveillance.

# Comment des introductions deviennent-elles des épidémies?

Parfois, ces introductions n'entraînent pas de cas secondaires ou d'épidémies localisées - en particulier si le cas index (la première personne à avoir été contaminée par le virus dans cette région) est détecté et isolé rapidement.
Souvent, cependant, le virus se propage sans être détecté dans la population locale jusqu'à ce que l'épidémie atteigne une taille significative.

Vous trouverez ci-dessous une réflexion montrant comment cela s'est déroulé à Wuhan. Sur l'axe y, nous avons l'espace; sur l'axe des x, nous avons le temps. La zone ombrée représente le nombre de cas.

"Le COVID-19 à Wuhan est passé d'un cas index en ~ novembre 2019 à plusieurs milliers de cas à la mi-janvier 2020, passant ainsi de l'introduction initiale à une transmission locale généralisée en environ 10 semaines. Nous pensons que les événements d'introduction internationaux ont commencé à se produire à la mi-janvier. Nous avons donc un délai critique de ~ 10 semaines de ce moment là jusqu'à la fin mars pour contenir ces épidémies naissantes avant qu'elles ne deviennent importantes." Ces nouvelles épidémies peuvent, à leur tour, semer de nouvelles introductions dans d'autres endroits.

Nous pouvons voir un exemple clair de cela dans les données de l'État de Washington. Passer la souris à racine du clade principal montre que le virus a probablement été introduit dans cette zone entre fin janvier et mi-février 2020 (méthodes).

À la mi-mars (~ 6 semaines plus tard), nous constatons qu'une épidémie locale bat son plein dans cette zone. À partir des seules données de séquence, nous estimons un taux de doublement de 3 à 6 jours, en supposant une population en croissance exponentielle.

Nous pouvons également voir un exemple de la façon dont des épidémies initialement localisées ont introduit des cas dans d'autres endroits. Les échantillons de l'Utah près du sommet de l'arbre (en orange) se nichent dans le clade principal de l'épidémie de Washington. Cela correspond à une introduction de Washington à l'Utah, bien qu'il soit également possible qu'il y ait eu des étapes intermédiaires dans la chaîne de transmission.

Nous ne savons pas avec certitude si cette introduction entraînera une épidémie locale dans l'Utah. Mais, si le virus continue de se propager après ces introductions, nous nous attendons à ce que cela se déroule au cours des 4 prochaines semaines.

Ce n'est qu'un exemple. Il peut y avoir eu d'autres introductions dans l'Utah (ou d'autres régions) que nous ne pouvons pas voir sur l'arbre parce qu'elles n'ont pas été séquencées. Plus important encore, les zones qui ne sont pas encore au courant des épidémies locales devraient se préparer maintenant et commencer les tests de surveillance.

Suivant cette même logique, il existe probablement de nombreuses chaînes de transmission locales que nous ne connaissons pas encore.

Cela signifie que même pour les régions qui viennent de mettre en œuvre des mesures de distanciation sociale, nous devons nous attendre à ce que le nombre de cas augmente au cours des prochaines semaines. Ces augmentations n'indiquent PAS que les mesures ne fonctionnent pas. Il faut juste du temps pour que les personnes déjà infectées (et éventuellement les membres de leur foyer) manifestent des symptômes, soient traitées et se rétablissent. Nous nous attendons également à une augmentation du nombre de cas signalés à mesure que les tests deviendront plus largement disponibles.

Il est essentiel que la distanciation sociale soit maintenue pendant cette période. Comme indiqué à droite, le nombre de cas continue d'augmenter après la mise en place de l'intervention. Mais, avec le temps, le nombre cumulé de cas diminue fortement.

## La distanciation sociale n'arrête pas tous les nouveaux cas du jour au lendemain, mais elle permet de réduire considérablement le nombre de cas et de décès au fil du temps

La distanciation sociale - c'est-à-dire la diminution du nombre de personnes que vous rencontrez chaque jour - cela peut être difficile, mais cette mesure est extrêmement bénéfique pour le bien public.  
 Si chacun diminuait ses contacts quotidiens de 25 %, on pourrait s'attendre à une diminution de 50 % du nombre cumulé de cas au cours du mois suivant ([Klein et al., 2020-03-13](https://institutefordiseasemodeling.github.io/COVID-public/reports/Working%20paper%20%E2%80%93%20model-based%20estimates%20of%20COVID-19%20burden%20in%20King%20and%20Snohomish%20counties%20through%20April%207.pdf)).

  • Le virus a été introduit à plusieurs reprises dans de nombreuses régions du globe.

- Nous voyons des preuves de transmission locale dans de nombreuses régions; au-delà de ces constations, nous nous attendons à ce que les introductions précédentes aient déjà donné lieu à de nombreuses épidémies locales qui deviendront évidentes dans les semaines à venir.
- Il est essentiel de contrôler les épidémies locales par la distanciation sociale: - #FlattenTheCurve (Aplatir la courbe) pour éviter que les systèmes de santé ne soient submergés par de nombreux cas simultanés - Réduire le nombre total de cas et de décès - Prévoir du temps pour le développement de thérapies et de vaccins
# Mesures que vous pouvez prendre
## En tant que particulier
* Réduisez drastiquement le nombre de personnes avec lesquelles vous êtes en contact chaque jour, surtout si vous faites partie d'un groupe vulnérable (par exemple, les personnes âgées et les personnes souffrant de maladies préexistantes).
* N'oubliez pas que même si vous ne faites pas partie de ces personnes vulnérables, de nombreuses personnes autour de vous le sont ; suivez ces pratiques pour protéger les autres.
* Lavez-vous les mains "comme si vous veniez de couper un piment et que vous deviez changer une lentille de contact".
* Restez à la maison si vous êtes malade ; soyez prêt à vous procurer quelques provisions supplémentaires au cas où vous auriez besoin de vous mettre en quarantaine.
* Si vous êtes un employeur, encouragez vos employés à rester chez eux lorsqu'ils sont malades (et soutenez-les financièrement).


## En tant qu'autorité responsable
* Rendre les tests gratuits et largement disponibles.  
* Mettre en place des mesures de distanciation sociale.
* Financer et mettre en œuvre de vastes efforts de "contact tracing" (traçage des contacts).
* Soutenir financièrement les personnes touchées par les mesures de distanciation sociale (par exemple, les travailleurs payés a l'heure, les personnes ayant des responsabilités de soins aux personnes âgées ou aux enfants, les petites entreprises, etc.).

Nous tenons à souligner le superbe travail accompli si rapidement par tous les scientifiques impliqués dans cette épidémie, et en particulier ceux qui travaillent en Chine. Ce n'est que par le partage rapide des données génomiques et des métadonnées que de telles analyses sont possibles.
