Chapter 5 - "Registered TodoServer" - can only start one instance

I am not sure, if that is intended, I wouldn't have thought so, but the way the (registered) TodoServer is implemented in code_samples/ch05/registered_todo_server.ex results in every instance creation (after the first one) in an error, as the "the name is already taken, or it has already been given another name" - all messages get then send to the first server.
