People getting stuck / queue doesn't start

Seemingly with no error in the logs, people get stuck in the queue (uploaded a picture) or the queue doesn't start at all. Fixed by a server restart, map change doesn't seem to fix the issue for me.

Were you using the included server.cfg and ensuring no other cfgs were executed at any point? Off the top of my head this looks like an issue with more people being in the server than are allowed into the 2 teams, so after they join and cant be added to a team (because of team size), they're no longer tracked in the queue and not in an arena. Also, does the provided requeue command fix it?
