Created a fork with two extra features

Hi @rickpastoor,

I took it upon myself to fork this extension for my company, as we really liked its simplicity and speed but needed two extra features:

• post-estimation points (another type of rating for each card)
• visible card IDs.

The fork is here and the published extension is here.

I would normally have made a pull request to this repo, but for the sake of simplicity, I removed your separator card feature from my fork - so it's probably not mergeable as is.
However, perhaps you can use parts of my code to add these features to the extensions if you find them interesting.
