Providers tab of admin view should list provider information

Providers tab of admin view should list provider information including Provider NPI, Provider Type, Provider Ext, Provider Name, Tax ID, Phone, and Team. The provider list should be able to be filtered by typing a provider name into the filter box located under the tabs. This filter process should occur automatically as the user is typing.

Clicking on the "x" to the right of the filter box should clear out any typing and revert back to the default grayed text "provider name" as follows:
