@quincylvania Recently added one for the osmus tasking manager (which was derived from the HOT tm). That's here if helpful.

There are a number of online wizards for generating these now. Essentially you just need to specify what user data is captured (e.g. OSM account info, email addresses, IP addresses). We can work through this together if that would help.
