Add Stage 2 formatting parameters to the URL page

Allow for formatting parameters from muse2ps to be set from the page. Probably underneath the Generate PDF button. Start with the music font sizes (6, 14, 16, 18 and 21) as a select menu, default to size 18, perhaps:

• size 6 = staff height of 0.08" (2.032mm)
• size 14 = staff height of 0.1867" (4.74mm), for small score size for large ensembles
• size 16 = staff height 0.2133" (5.4867mm), for small piano size/medium ensemble size
• size 18 = staff height 0.2400" (6.096mm), for large piano size/small ensemble size
• size 21 = staff height 0.2800" (7.112mm), for orchestral parts

Other layout options can be added as desired/needed.
