Logo/Title visible for server

Is your feature request related to a problem? Please describe

When logging into different notify servers, there's not much to visually distinguish them.
This could lead to issues for support staff who work accross multiple servers incorrectly configuing things for the wrong server/client.

Describe the solution you'd like

Something in the bottom area showing the install information would be sufficient?

Potentially this could be configured in the YAML file or a database table or something similar?

A logo would be nice too, but not sure where it would best live...

Acceptance Criteria

• There's somewhere to tell which server you're on without having to look at the URL

Nice to haves

• Updated Title in the browser
• Logo visible
• View in bottom of screen showing site
• Chat bot says who it is (e.g. Hi from Notify {{ site_name }})
