Bug Report

Description

The current multi-choice option when making a Question Template does not function properly in any language other than English as the input boxes for the options will not show up and will vanish when attempting to change the language from English.

Steps to Reproduce

  1. After logging in the dashboard, in the sidebar, click on Performance and then choose Question Template from the sub-menu.
  2. Create a new question template.
  3. Insert the title and company for the question template and press save.
  4. In the add a question section, choose multi-choice for question type where it should display the four different input boxes for the multi-choice.
  5. Change the current language from English to any of your choice and the input boxes for the multi-choice will disappear.

Expected Behavior

When attempting to create a multi-choice question type while making a new Question Template, the input boxes for user input should show up regardless of the current language chosen.

Actual Behavior

When attempting to create a multi-choice question type while making a new Question Template, the input boxes for user input will not shown up in any form unless the current language is English.
