insertMenuItem bug #17

The implementation for insertMenuItem does not call pokeFMask. As a result, calls to insertMenuItem always fail.

Also, the implementation needs to use withTString not withTStringLen.

Also, the pokes in the implementation of withMenuItemInfo have no effect, as if they're not being compiled.
