Error message regarding Node.js & code: 128 & outputData

Thanks for this amazing API, when I followed all the steps via your README.md, I have no idea why these error message showed up when I manually click action. I've already included the start & end text in my README. It just doesn't work for me. I tried to figure it out yet fail, if you could provide some clue when you're available that will be amazing, thank you.
