API for Captcha

Im a bit confused.

The documentation says to place code in my form but when I do it kills the form.

Im not even sure where my form is because the one I created which resides in /published/myform.php does not even contain any form fields.

The instructions are not very useful in they don't indicate what the actual form file is or where it is located?
If I don't know where it is, I can't very well add the code to call the API?
