Multimarkdown tables extension

Context

I encounter the need to merge certain header cells, and other cells in tables. This is very common in technical tables.
On vscode it renders properly, like multimarkdown tables, but with markdown-it-py, it doesn't.

Proposal

I was hoping someone who knows how to port stuff could port this multimarkdown table extension that's been done for the markdown-it javascript implementation.
