Publish the Vaccination Context Schema on /credentials/vaccination/v1

The current vaccine QR code relies on two linked entities:

• https://www.w3.org/2018/credentials/v1
• https://cowin.gov.in/credentials/vaccination/v1

The first is already published at the expected URL, but the second isn't. As such, the code contains a custom document loader.

This, and any other implementations that rely on the Vaccine QR will be simplified a lot if the JSON doc from the NPM package were to instead be published on the proper URL.

That way, any implementation could just expand the doc as per JSON-LD without having to implement a custom document loader.

With the new certificate using https://cowin.gov.in/credentials/vaccination/v2, the same issue still applies. The URL returns a 404, resulting in additional burden on the verifier end.
