URL: zero based index to 1

Issue description

using 0 in the URL might be irritating
We could simply shift it by one to make it easier to interpret by the human mind.

Steps to reproduce

Use <Url/> inside <Plugins/> (requires #12)
or set <Config url="hash" /> (requires https://github.com/sinnerschrader/dekk/releases/tag/v1.0.0-4)

besides readability, this has no impact on using Dekk.

I think because Dekk is intended for people with a technical background they should decide what they want to do. So if you do it: Make it configurable or allow to change it via a custom plugin.
