Modify get_hipaa_disclosure_log_from_ehr_fhir_logs() to also return these values immediately after ehr_id

  1. redcap_ehr_settings.ehr_name
  2. redcap_ehr_settings.fhir_base_url
  3. redcap_ehr_settings.patient_identifier_string
