Add Media in WordPress Post not working

I can upload files to the media library with no problem, but can't insert images into posts or pages. So you know, this is on the 3.3.1 version of WP as well. 3.5.1.

I have more detail from FireBug:

HTTP Error 404.15 - Not Found
The request filtering module is configured to deny a request where the query string is too long.

Note - this does appear when clicking on Add Media, but on the page load.
