A decent first party contact form

I know you could consider this plugin territory, but it's the one thing that everyone asks for; a decent contact form that sends a copy to the subscriber. Preferably one that supports multiple fieldtypes and a decent customizable mail output plus a database connection, so you can use it as a subscription form as well.

This would be even better if it allowed the main contact recipient to be dynamically determined by the associated entry. For example, a company with multiple locations might need the same contact form for each branch but with a different contact person unique to that branch. It can be frustrating for clients to have to use plugins to make the same form over and over again for each location they add, so having a way to automate this would be fantastic.
