Add mechanism to create scopes/collections/indexes/etc

In order to improve the experience of setting up a test environment, we should support helpers/tools for creating much of the initial state required by our tests. In addition, allocate definitions should be extended with an "initial-setup" block allowing you to predefine the desired items.

Need to consider how we create primary indexes on capella since I don't think you can run index management queries there.
