Teacher account creation behavior

• Remove db echo that appears at the bottom of screen as user fills in form.
• When user clicks submit, they should get a notification that confirms their responses have been recorded and they will receive further information on setting up their account. (Right now, the form box stays open with all the information you filled out, with no obvious way to close the box or what to do next). If Jon can create the dialog that appears after submission, Pat can update the text that gets displayed.
