Governance review request form

Spins off from #474

Problem:

• We (TAG CS members) want to make governance reviews, but we don't have enough bandwidth.
• We want projects to do some easier and time consuming tasks for the review.
• For that reason, we wanted to have projects to do a self-assessment before asking a review from us.

However, given that CNCF TOC is redefining the criteria for projects moving levels, we wanted to keep the self assessment work on hold for now as it is not a good time to work on it until the refinement has more progress.

We still want the projects to do some data collection for us though. Things like:

• Where is the governance document?
• What is the website?
• Where is the contributing guide?
• ...

This will be an interim process + document.

Output of this task will be a GitHub issue form.

Note: I just did the form for POC in a temporary repository. Let's collaborate on the Google doc and I will update the form in the end.

I think this all looks good, and I'm in favor of rolling this out into a GH form now and iterating on it as we learn what works / doesn't for people. We can use the Knative governance review as a test of how well this works.
