Follow up to #40021

  • For Report a broken site, after deleting contact information from the form and resubmitting, contact info still appears in the form.
  • add a new line in the Submit report panel above the Submit button, right before the Learn More link: "If you provide contact info it will be stored for future reports."
  • Keyboard Enter doesn't work. For now is required to tab thru or use Mouse to press Submit button
