Describe the task
Create the frontend visuals for the file upload page (heavily based off of better berries with minor changes).

Acceptance Criteria

New Calculation button present
Upload nmp file button present
Disclaimers added

This ticket has been combined with ticket Add file upload functionality #65 as they were closely related.
There still needs to be styling done in a future ticket as currently the two buttons and disclaimer are all that are being displayed.
