Manager Checklist:

• Get the right people in the same place and start sharing information
• Start sharing information with stakeholders (higher management, customers, marketing, etc). Keep doing so regularly.
• Start brainstorming solutions
• Start applying low-hanging-fruit (low risk; easy/fast to try) solutions
• Continue until the problem is resolved
• Have (or schedule if it's off-work hours) a retro to determine root cause and future prevention steps
