PL/Series doesn't show properly and displays videos from other channels

I plan to use Programs to organize different video series under the same channel. Once the Series is created from the Program, in the Organize Program page it will show the videos under that Series, but when I edit that Series, it shows "0 videos", and when I toggle on the Add to first page, only the title of the Series will show up on the first page, the videos are not. However, if I suggest an arbitrary video (even from other channels) it will show up in both Suggested (Featured in my video) and under this series.
Additionally, the Program(PL) list doesn't show up under My Channel like it does in your instruction gif.

I am just very confused.

can you please try to clear the cache?

Both my local and site's cache has been cleared (and the cache plugin isn't enabled) I can attach video if needed.

The other dev tried the same steps and got the same result.

Video shows clearing cache using admin account, inspecting the Series setting via uploader's account and the At the Ready list shows there's 0 videos (in setting view, homepage and channel view) However if you click into the PL/Series there are indeed videos under it.

let me see if I undestand, the issue is because here appears 0 videos?

The issue is

1. It appears 0 videos in your screen shot.
2. It appears 0 videos on the playlist cover anywhere else (There's indeed 2 videos in it)
3. When "Show on first page" is enabled for the PL, Suggested videos not from this PL is also displayed under that section on first page. (This has lower priority, we may not use this feature at all)
   
   Images shows 0 on the cover and the video on the left is not in this list.

There appears to be database errors but we are unable to find the corresponding error log.

the problem is, I could not reproduce it locally

1. try to git pull to see if this is a missing update
2. if it persists, please try to reproduce it on demo site https://demo.avideo.com/

Can you remind us how to access/enable logging for db errors for the site? There might be some issues there but we couldn't find where it's configured.

I did a core file overhaul and it seem to have fixed some of the issues and we fond permission inconsistency that's causing the log to not show up. Thanks for the help.
