Add relevant project/response name to New Account Request emails

This would be a minor QOL enhancement that I keep forgetting to bring up. It'd be great if I could see with which project/response the individual request emails are associated. I usually have to go into the admin and play with the filters to find the relevant request, not a big deal, but having the associated project right in the email would save a bit of time for everyone--especially for us admins and sponsors who manage multiple projects.

Also add account requestor's name to emails.

Maybe. There was a conscious decision at one point not to. One reason was b/c there might be multiple and we didn't want to deal with that at the time. The other is we don't know where these emails are going sometimes so do we really want to be flying names all over the place? Maybe that's trivial but we've tried to be on the cautious side with this.
