Method to Report Hackers via Bandwidth

Reason

I've been seeing some people in the Discord server ask about reporting hackers. There's been threads/instances like this one and this one. It appears the current method is just DMing one of the moderators. I feel like it could be helpful to have a slash command that will put all hacker reports in a private moderator channel so it can be a little more organized.

This is something that I personally would want to work on, but I wanted to post an issue first before doing it!

Proposed Tasks

• Create a modal to allow users to submit text details/hacker PNID (and images too, Discord modals can accept files)
• Create hack_reports_channel row in database.js
• Have Bandwidth post the report in the designated channel for moderators to review
• Possibly advertise/document Bandwidth's commands somewhere so more people know about them
