There are some empty table headers (shown as errors in the WAVE tool): View all responses > Summary: question type "Rate (scale 1...5)"
Additionally, some table rows in the questionnaire are empty.
Question type "Rate (scale 1...5)": In the corresponding table, the average rank is not correctly assigned to the column headings. Columns labeled with 1, 2 and 3 are read as "123" by the screen reader.
