OCC post conference survey

It would be great to have a survey ready to ask participants to fill out immediately after the conference. Anyone wants to draft one in google forms? @xolotl?

@dasaderi I started a draft post-event survey in a gdoc so we can edit freely and move to Google Forms when finalized.

Thank you @xolotl for putting this together. I made a couple of changes in the form directly. @rchampieux, would you like to review it? The survey form is in our shared folder. I will also ask Nick Shockey if he would be willing to review it. My goal is to have it out in the next couple of days.

@ashleyfarley, we should probably use the openconcascadia email for this. Would you be willing to send it if I drafted the content? Thanks!
