Does not centre in right spot when opened from .qgz or .qgs file.

Whenn QGIS opened first without project the scale and center coordinate is fine.

This could be a barrier to collaborators not familiar with QGIS as they would need to zoom to feature such as watershed group.

Any ideas @smnorris ?

I've noticed that too, it is weird and disorienting.

Maybe using the "Set Project Full Extent" dialog in "Project Properties" - > "View Settings" would help?

Nope.
Even if I create a project from scratch, add a single postgis layer, define the project extent and save, when opening the project file the map view is centred at 0,0 at 1:13.

Might be a mac specific bug: qgis/QGIS#45809
