Request Service Creation Error

When the user follows the steps below to create initiative and request service creation:
The user clicks on Tools > Open under Onboarding Form > Save > Open the created Onboarding Form > Enter Initiative acronym and Create Initiative

The initiative was successfully created. But when the user clicks on 'Request Service Creation' under Initiative Storage, the DataHub admin doesn't see the request for approval.

Additionally, after clicking Save on Onboarding form, an error page loads

The error happens in both the dev and prod sites.
