Add biographical data source

Purpose

Allow the user to add biographical data (or maybe multiple sets of biographical data, or maybe that's a data source parameter) to a workflow. It should appear just like any other sensor.

Display

• When the card appears, the contents should just be the data form, rather than a capture button, etc.
• We'll need a "biographical data" icon for the modality area and collection items.
