It would be cool to have support for two simultaneous mice to play 2 player Settlers or 2 player Lemmings. I think Linux already supports multiple mouse inputs, but the information doesn't make it into the core.

The whole MiSTer logic made around a single mouse and single keyboard. You request to redesign MiSTer input system to support just 2 games in one core.

To side with @rsn8887, technically, there are many arcade cabinets that are 2 or 4 player capable. I'm not saying it would be simple or quick, but then the real Amiga has two 9-pin ports for mice/joysticks. We're not talking about some parallel port contraptions that are providing support for 4 players.
